HOW TO APPLY

• In person:
  Stop by the building with a completed application (you may Download), copies of your ID and Social Security card and proof of income (paycheck stubs, W2 or tax returns). We will charge you a non-refundable fee of $20 ($10 extra per additional adult applicant). Payment can be made cash, cashier’s check, Money Order, Visa, Mastercard, American Express and traveler’s check (no personal checks).
  
• By Fax:
  Fax us a completed application (Download) along with a copy of your ID, SS card and proof of income. Include a credit card authorization form (Download) for the processing fee and a copy of the credit card used.

• By Mail:
  Mail us your application (Download) and required documents. Include a money order for the processing fee or a credit card authorization form (Download). If you use the credit card option, include a copy of the credit card you wish to use.

• Online:
  For security reasons, we do not process online applications at this time.
